Our commitment to your health doesn’t end after you leave the hospital. The goal of Stoughton Hospital’s Home Health Services is to provide, safe quality healthcare in a personalized manner in the comfort of your own home.

Our Home Health Services can be arranged with one phone call. Our staff will communicate with your physician to provide a continuation of needed services.

During home visits, our professional nurses and therapists will educate you and your family regarding how to manage your illness. Our staff will address specific problems you are facing, teach skills to speed recovery and reduce the possibility of re-hospitalization, and provide support and education in managing chronic illnesses.

Our caring staff will also offer support and education to help spouses, families and friends cope and adjust to new roles as caregivers. Depending on individual needs, the program may include training in modifying the environment, assisting with activities of daily living, home adaptations and equipment, safe transfers, lifts and/or repositioning.
