Stoughton Hospital Home Health has been named a Top Agency of the 2016 HomeCare Elite®, recognition of the top-performing home health agencies in the United States. For more than ten years, HomeCare Elite has annually identified the top 25 percent of Medicare-certified agencies and top 500 agencies overall.
Home health agencies are ranked by an analysis of publicly available performance measures in quality outcomes, best practice implementation, patient experience (Home Health CAHPS®), quality improvement and consistency, and financial performance. In order to be considered, an agency must be Medicare-certified and have data for at least one outcome in Home Health Compare. During home visits, our professional nurses and therapists will educate you and your family on how to safely stay in your own home. Our staff will address specific problems you are facing, teach skills to speed recovery and reduce the possibility of re-hospitalization, and provide support and education in managing chronic illnesses.
Our caring staff will also offer support and education to help spouses, families and friends cope and adjust to new roles as caregivers. Depending on individual needs, this may include education in modifying the environment, adequate nutrition, complex wound management, assisting with activities of daily living, home safety and medication management.
