Stoughton Hospital Home Health staff will address specific problems you and your family are facing, teach skills to speed recovery and reduce the possibility of re-hospitalization, and provide support and education in managing chronic illnesses. During home visits, professional nurses and therapists will educate you and your family on how to safely stay in your own home.
Caring staff will also offer support and education to help spouses, families and friends cope and adjust to new roles as caregivers. Depending on individual needs, this may include education in modifying the environment, adequate nutrition, complex wound management, assisting with activities of daily living, home safety and medication management.
